Auto-checkout macros for cash patients

This behavior has been improved to ignore modalities that do not have CPT codes associated. Only cash patients were presenting the procedure pick list due to the absence of a third party payer.

Account type catalog

The legacy "responsible party" fields have been removed. They were present as the result of a legacy data conversion. The Insurance Policies window implements this behavior.

Colorization of Insurance Policy window

Additional fields required by the Insurance Manager pre-billing report have been colorized as dark navy blue when required, particularly if relationship to insured is not "self".
