CMS-1500 for revision 08/05
The CMS-1500 form revision 08/05 is complete for distribution in the November 30, 2006 release.
Concurrent support is provided for the existing CMS-1500 form revision 12/90.
Migration utilities are included to provide a phased transition to the new form on a payer by payer basis or to change all payers globally to the 08/05 form.
