DX and CPT catalog browser;

The browser has been modified to show active entries only be default with a pair of radio buttons to toggle between "Show Active Only" and "Show All" entries.

Appointment Report;

The appointment report has been modified to allow the user to choose any particular active provider, or choose all providers.  The provider selection will remain the combobox selection for subsequent use.
