MyEMR text engine upgrade

Diagnosis pattern window now supports up to 12 ICD codes

The diagnosis pattern window has been modified in support of  the 12 ICD codes.   Some Medicare localities are now requiring 10 diagnosis codes to bill a 5 region chiropractic adjustment.
