Blank diagnosis record

Diagnosis text generation date consideration

The behavior of the DX generation looks at the date/time stamp to determine if the DX should be generated.  It was previously looking at the date modified, so if the pre-existing diagnosis was disturbed in any way, the diagnosis would generate, undesired,
